Ring tone and volume change without adjusting them

Since the last update, at some point during the day while my phone is on, my ring and notification volume lowers to nearly inaudible. But when I check the ring volume, it's still where I set it, almost all the way up. Adjusting ring volume with the volume buttons on the side of the phone does nothing to affect the volume even though it appears to be visually adjusting. Restarting the phone fixes it temporarily until at some point a few hours later, ring volume and notification volume is back to near inaudible. Anyone else experiencing this or has a fix? I'm guessing it's the last update or an app causing the problem.
 
It happened again today and I went into safe mode. Trouble is, I think your phone restarts when you go into safe mode and restarting my phone temporarily fixes the issue. Plus, in Safe mode I can't send myself a text to see if the actual volume i hear, correlates with the volume setting on the phone.
 
I suggest using the phone in Safe Mode for the better part of a day (if feasible) to see if the problem recurs when other people call or text you. Waiting to see if it happens for a only a few minutes may not be enough to determine if the problem is due to some 3rd party app vs the system.

EDIT: I read through that support thread, and it looks like at least some people said it still happens in Safe Mode. It's still worth a try on your end. I also have the random reduction of ringer volume from its normal value of 6 down to 2. Also, while listening to audio over Bluetooth in my car, sometimes media volume will randomly drop to zero, making me think that my stream stopped, but when I look at the phone, it's still playing.
