Ringtone continues playing after ending call

I have the craziest damn bug you've ever heard... I have a ringtone that plays when a call comes in, answer the phone - the ringtone turns off, as expected. However, after I hang up, the ringtone will continue to play. I can adjust the volume, but since it's a ringtone, I don't know of an app to open to change it, other than opening my phone, calling voicemail, hang up, and still the ringtone persists (good thing I like the song — Brian Setzer and Jeff Beck's version of "Twenty Flight Rock".)

Powering off the phone is the only thing that can make it work and it seems to occur more often when the phone is charging (charged?) and plugged in (vs Qi charging).

Have you tried clearing the cached data in Settings, Storage....or a Cache Partition wipe? Possibly even Safe Mode?

Yes, I've done both clearing of the cache from the Settings > Storage, and a System Cache wipe from the Recovery Menu (which helped clear up a bunch of annoyances, particularly with Google Maps), but this occurred after both of those were cleared.

It sounds like some other player might be playing that file? Are you using the stock dialer? Rooted? Different ROM? At least it's a snappy ringtone.

Not rooted. Stock Android.

Wouldn't it show up in the running apps if something else was playing my ringtone?

It should. Maybe it's a carrier issue? Maybe they're not registering that you're answering the call and continue ringing your number.

Have to tried rebooting the phone?

My LG G3 on Sprint does the exact same thing. My phone is not rooted and is running Android 5.0.1. Every time I get a call, after hanging up it continues to ring without stopping. Sometimes a random action in the settings or camera will shut it off, but the only reliable method is to restart it. I sure wish we had an answer on how to fix it!
