Samsung Gear VR / S2 Verizon Offer Submission issues

Now that our S7 and S7E have shipped, I downloaded our Verizon Equipment Invoice to submit the Samsung Gear VR / S2 Verizon Offer claim form.

While completing the form, I read the qualifications and found at the end that "the name on your receipt must match the name on the Offer Claim Form." However, our name is strangely absent from our Verizon receipt.

I am not going to submit the claim form without the proper documentation, as it would most likely be denied.

A quick call to Verizon CS was unable to resolve the issue. The CS Rep did advise I should be able to stop by a Verizon store to get a printed copy of our receipt with our name on it. I'll post back tomorrow what I get from the store.

The invoice sent with the phone should have your name on it.
 
Typically the receipt in the box should have our name on it. But, then again, so should the copy available online. I'll find out soon enough.
 
A phone I ordered last year online came with a receipt in the box with ship to: which had my name and address. It had the location code, order #, typical receipt stuff with everything itemized and priced.

I stopped by a Verizon store today, but they could not provide a copy of the receipt with our name on it either.

Guess I'll have to wait until Wed evening to submit our claim form.
 
My receipt does not have my name on it, nor do I expect it to. I am certain Samsung will accept receipts from Verizon considering it is a Verizon promotion and they have an expected receipt format.

I wouldn't worry about it. They are ultimately looking for the NAC address, device id and that id matching on the receipt.
