screen rotation and heart rate sensor galaxy s5

screen rotation and heart rate sensor only work for about 5 min after resetting phone. I tried factor reset and that didn't fix problem. Does anybody know of anything that would cause this problem. I thought maybe gyroscope was bad , but why will it work off and on? Please any help will be appreciated.

If factory reset doesn't works, I would run for the warranty of the phone. :/
