SD card ejected and random freeze

For some reason, my SD card keeps getting dislodged randomly. The message appears "SD card is safe to remove" then later another message appears "Preparing SD card". Also, I get random freezes where whatever app I was using still works like if camera, it still changes if I move the phone but nothing is responding. Neither touch nor the power button responds.
If it helps, I first noticed this when I switched to an LTE SIM. Any ideas on what's wrong or how to fix it?

Welcome to Android Central! Does this happen with any SD card, or just that one? The card itself might be corrupt or defective. If this happens with all cards, there might be a problem with the SD slot--make sure it's clean by blowing it out with a can of compressed air.
