Sending in the Note 10 Plus for sammy trade in. The PDF form mistake

Some are saying the PDF automatically checks off that the phone doesn't turn on and that the screen is broke etc by default. What are you guys doing with the form? use white out over it?

Apparently Samsung fixed it. I just downloaded the shipping form.
