Setting the accessibility shortcut "off" does not remove the pesky screen icon in 5.1.1

The accessibility shortcut screen icon (not a tiny icon on the top) is full screen icon of a person within a circle on a translucent square. Settings>General>Accessibility/Shortcut key is OFF, but the icon does not disappear. It CONTINUES to be a g*ddamn nuisance - incredibly annoying. I hate that MF!!!! Please tell me its an Android 5.1.1 bug that will soon be fixed, or tell me what to do about it.... This is a seriously resistant 5.1.1 problem. Thank you for reading my rant.

Have you tried rebooting your device after you turned it off?

Have you tried long pressing the shortcut to see if it can be removed that way?

Might want to try wipe system cache via recovery mode.
