Update: Google is fixing this in Android M. Problem solved!

Android M:
You'll be able to lower your media volume via the volume controls in Android M.

Android Lollipop:
Anytime I want to play an android game without sound I'll click the "volume down" button and bring the volume all the way down to zero. Then I'll open the game in, for example the office or church (where I don't want it to make sound) and then the game music plays blaring at full volume.
Why? Because when I thought I was turning the volume down, I was actually turning the notification volume down. The volume button only changes the "media" volume AFTER there was a sound played (i.e. after everyone's looking at you).

Here's one way Google can fix this: make the volume button always change the "media" volume and provide a way to change the notification volume on the screen.
That's my proposition, let me know your guy's ideas on how to fix this problem below, and whether or not you're also affected by this problem.

A side note,

How to turn down media volume BEFORE you open up that loud game:

Simply clicking the volume down button before you open up a game will not make that game silent.

In order to do that go to Settings > Sound & notification and turn down the media volume.

You mean like this? The primary control changes based on what you are doing with the phone.

That doesn't look like android lollipop, right now android lollipop doesn't have any sort of "more sound options" AndroidLollipopSoundOptions2.png like the one you're showing. Even the "none" option (as shown in the picture in the first post) doesn't turn off media volume- only notifications.

Google should really implement volume control for notifications and media separately when you press the volume buttons, custom ROMs have been doing this for years. Even Windows Phone has implemented this!

But on a side note your Nexus 7 seems to be running on an older Android launcher, if you want to enjoy all the visual tweaks of Android Lollipop, download the Google Now Launcher:

This affects me too. Although sometimes I'll also get a bug where the volume rocker changes media even though its been minutes since media has played and no apps are open.

When I sit and think about it, I rarely every want to change my notification volume. I either want to switch between On/Vibrate/DND, or I want to change media volume. Maybe it's just me, but who wants to tweak their notification volume often? Maybe there are people that want loud ringers in some scenarios and quiet in others on a frequent basis? If there are, I've never met any of those people.

Loud when you're outside walking around a noisy area or at a bar, quieter when you're sitting at home.

Eh, I guess. Even at max volume I know I wouldn't hear it in a bar. I'd rely on vibrate. In fact I always have my phone on vibrate. I guess if you're a girl with a purse vibrate wouldn't do much good, but would you be able to hear it through a purse in a bar?

What if there was an auto-volume setting to adjust ringer volume based on ambient noise, similar to how displays have auto-brightness. Actually I'm sure there are apps for that.

Yes, I just did that, it looks a lot better.

I think Google tried to make the volume control simpler and smarter, but it lost too much functionality.

Wasn't this just implemented in KitKat? Why take it away? Have you checked settings>sounds? Maybe they just took it away on the fly.

Hopefully this info helps others like me who have been struggling with this same issue.

The solution is a free app named Rocker Locker.

But it hasn't happened since I've downgraded to KitKat.

EDIT:

I just realized that this thread is about lowering the ringer volume and thinking that you're lowering the media volume. My issue was lowering the media volume by going to Settings -> Sound and even then being a victim of the media volume suddenly and randomly going to Maximum.
