UCCW is grid & tiles by Alex T on play store and wallpaper was found on the wallbase app.
Also used apex launcher.

I've narrowed myself down to a single screen. The folders make it easy to get to my most-used apps. Just wish I could re-size the built-in flashlight app so that I didn't have to download a 1x1 widget.
