It's been about 4 days that my Note has stopped vibrating randomly. I have to turn the phone off and turn it back on so it will vibrate again. Any tips on how to fix that?

Any power-saving modes on? If so, turn them off and see if that helps.

If not, then does it vibrate in Safe Mode? https://support.t-mobile.com/docs/DOC-36242. If so, then something you installed is probably interfering. You may have to uninstall apps one by one until the problem goes away.

Thank you so much ! Yes I just got an update for the "phone app / dialer" and that's when it started doing it. But I haven't found a way to uninstall it and redownload it. I just cleared the data but it still does it. Before that got updated I didn't have an issue.

And it shows 3 phone apps

Try wiping the cache partition: https://support.t-mobile.com/docs/DOC-36229

The 3 Phone apps are probably just poorly labeled. On my Pixel 2 XL, there are 3 phone-related apps -- Phone (the main app), Phone and Messaging Storage, and Phone Services. I suspect your 3 Phone apps are related.
