Threats and Facebook app disappearing from folder - S5. What can I do?

"Threats" and Facebook app disappearing from folder - S5

I have a Samsung S5, ~14 months old.

In the last couple days, I have received a couple pop-up notices of "threats" to my phone. I can click Cancel or Okay - when I click Okay, nothing comes up.

However, my background has gone back to the original Samsung background, and I have "lost" my Facebook app in a folder I created for my social media accounts. I can go into Apps and bring the Facebook app back to the folder, but I'm not sure why this occurs.

This is not the first time for the Facebook app to disappear on me.

My questions:

* How to I cleanse/de-bug/take care of these threats that are popping up?
* Why is Facebook disappearing on my phone?
* What else should I consider in this frustration?
