Trouble Flashing Clockwork Recovery

I am having trouble with Clockwork recovery. Whatever way I try to get it on my phone it doesn't "stay". I put it on through rom manager, flashed it through wugfresh's toolkit, and flashed it manually using commands. They are all "successful" as the first time a reboot into recovery it works fine. But the next time I reboot into recovery I get the little android guy with the red exclamation point. I do not know what is going on. Any help?

I don't know if it matters, but I am on 4.0.4.

1. Install Root Explorer from the market and delete /system/reboot-from-recovery.p – *Mount as R/W first*

2. Flash CWM w/ ROM Manager

3. You now have permanent CWM Recovery.
