I bought my GN4 in December of last year , and the battery life then was absolutely wonderful , I recall times where i didn't have to recharge for 2 days!

But now it's gone seriously awry .... After a full charge the phone , the battery dies within 6-7 hours on standyby with wifi or LTE , and in 2-2.5 hours playing games, or browsing . Even scarier, when the battery reaches the battery alert threshhold at 15% I usually have less than 2 or 3 minutes to plug it before it turns itself off . Sometimes it even shuts down at 11-12% , turns back on again and shows me that the battery level went back to 0%

It's just impossible to understand .

Here are the battery usage logs,

I'm an average user, not using too many apps , always keeping the device running smoothly and never letting it get too hot . Running Lollipop 5.1.1

After you updated to 5.0 did you happen to do a system cache wipe? I'd give it a try.

Have you looked at the apps consuming the battery?Sometimes a rouge or locked up app can kill a battery. I've has where something is trying to backup to my cloud, or the play store trying to update causing this issue.

Yup, time for a new battery. I bought my N4 on Halloween last year so almost a year old. Was having similar issues where mine would say something like 25% and then you look at it a minute later and it's at 1% and then shuts down before you can get the charger connected. I called AT&T and had them ship me a brand-new battery, no questions asked other than what it is or isn't doing. Battery and shipping was free of charge because it is still within the manufacturer's 1 year warranty. I would hope the other carriers would do the same. I also bought a brand-new battery from eBay for $20 shipped. Be VERY careful buying on eBay or Amazon because there are TONS of counterfeit batteries without NFC or even less mAh. I got a genuine factory sealed one.

Long story short, the new battery fixed my issues. 350ish days with a MINIMUM one charge per day kills the battery fast. Isn't it great that it's not a Note 5 with it's garbage sealed battery?
