Most of my USB flash drives work with the USB C adapter for my Pixel XL, but I have a 128GB Patriot Rage that doesn't. Could it be the file system, too large a drive, an incompatibility or something else?

I'm not really sure what to even try to fix it.

Thanks for any feedback.

Best bet would be to try it on another device.
Be that a borrowed phone, a PC, etc.
A PC would offer better options for troubleshooting, and checking the file system, formatting etc.
If it is empty, can you format it from the phone.

Quick search show a USB 3.0 drive, with USB A connector.
Is that right, and you are using a USB OTG adapter or similar.
One drive I saw on Amazon was by default formatted for a Mac.

Holy crap, I never knew that this was even a thing. Using the USB-C adapter that comes with the Pixel, insert a thumb drive into it then put it in the phone. Format it like the Pixel wants, then load it up. I've come over from having a Samsung phone for the past 6 years so this is groundbreaking for me.

I don't have much to add to your problem, but I love that you're having it - otherwise I never would have thought to try this. I just slammed in 5 different 16GB USB drives (different brands) and they all work flawlessly.

I then hauled out a 1TB Western Digital external drive and plugged that in. Pixel wanted to format it so I let it. After a few minutes it was readable and I could put media on it from my laptop and play it on the phone.

I would have to say, even after this SHORT little experiment, that perhaps it's that one thumb drive of yours that could be bad? Does Pixel even recognize it when you plug it in? It should come up with at least a message saying that it needs to be set up in a format the phone will accept...

I do this with my DashCam. I just unplugged the USB from the power plug, put the adapter on and plug it into my XL. very easy..
