Verizon Galaxy Note 2 - Music player deleting songs in playlist?

Today I had my note 2 do something weird, it took all the songs that are stored on my SD card out of every playlist but left the rest. It's not the first time it's happened but it is the first it happened without me touching the SD card. I tried restarting the phone, clearing the cache and data, but nothing worked. Any one have any ideas?

Had the same thing happen on mine. Seems that only the items i've downloaded from the play music store in the last week are in my playlist currently.

Had the exact same thing happen to me with my Note 3. 64 GB microsd card filled to about 46 GB with music. All the tracks are preserved, album organization, genres, etc. Playlists appear, but all are empty. I use Tunesync to transfer music and playlists from iTunes on my PC, but I don't think that's related.
