Volume w headphones in uses the phone icon instead of the music one, not as precise (Spotify)

I noticed recently that when adjusting Spotify volume when using headphones when the Spotify app is not on screen it uses the volume slider with the phone icon, which has larger steps than the music one. Any idea why this may be or how to fix it? Just checked and Youtube uses the music one, and Spotify does also when headphones are NOT inserted.

Also just noticed that when in the New York Times app while listening to Spotify it uses the music volume slider, but when in other apps it uses the phone one. ???

Still on Android 7.0 (Nougat), so not an Oreo issue.
 
When you're running a media app in the foreground, the volume control is the media volume. When you're in a phone call, the volume control is the phone volume. When you're not doing anything, the volume control is the ringer volume.

That's standard Android. (And it doesn't matter if the headphones are plugged in, you're using Bluetooth headphones or anything else.) If pressing one of the volume buttons brings up a slider and a meatball (like the Settings icon), tap the meatball and you should get a screen with all 4 volume sliders.
 
I am talking about the volume for music from Spotify using different sliders/adjusters depending on whether I am in the app or not or whether headphones are plugged in.

I just tested again - played music from Spotify without headphones in, went to home screen, adjusted volume, it uses the music volume (the one with the music note icon). Did the same thing with headphones plugged in and it used the phone volume slider (phone icon) to adjust the volume of the music. The phone one only takes three steps to get to half way up, the music one takes 7 so obviously that one is much better. Very strange. Don't think this has always happened.
