Started getting this yesterday. Everything is up to date. Any ideas?

Phone still shows connected by Bluetooth in notifications.

Note 4 on Tmo.

This happened to me today. Unfortunately the fix was not something I wanted to do. I uninstalled Gear Manager, reinstalled the newest version from Galaxy apps webstore, and when it reconnected to my gear it forced me to do a factory reset on my watch. I did that, and now its working. This happened after installing a update for the Gear S a few days ago.

Same here! Uninstalled gear mgr, then reinstalled. Factory reset watch. All is fine so far.
