What does this tell me?

Missdora.... I am worried you have a fake Note 3 and that would explain your issues. That menu looks very fake and nothing like any Note 3 I've seen (regardless of carrier).

Random idea.. Can you take a picture of the "about device" screen and post it?

The menu isn't fake. My note 3 that I got at a corporate store and my note 3 that I got shipped to my house from Verizon have the same menu. My note 2 and s3 had similar menus too. The tmobile menu is a bit different.

Also a fake is highly unlikely to have CDMA voice and 4GLTE on Verizon.

View attachment 107055 this can't be normal. 5 hours and down to 44% with only 2 hours of screen time?

It seems a bit low, my phone gets worse but I know that there is something wrong with mine. I am just waiting for kk to do a factory data reset

Yes my phone is not fake. I bought it directly from a corporate store

Do a full charge and let it run as close to 0 as possible and then take the screen shot for gsam again. 6% usage isn't really enough to determine much. Also based on your second screenshot, your radio signal is pretty bad at times.

What is the radio signal? My service area? I will try to do this today. I just always charge it when it gets low bc I can't go long with out a phone lol. But I.I will use it as a usual and then post screen shots once it gets really low

The orange line in the second screenshot

Click on the button on the bottom right of the first screen shot, and take screen shots of all the menus there too

There are people here who understand those screenshots far better than I do. I can say that your radio signal looks far different from mine. If the problem is the signal, I believe there are apps that you can download that will alert you when you are entering a dead zone. There are also apps that can turn off your radio signal when you enter a dead zone. You might find some helpful info here: signal - Does 'poor' reception deplete the battery quicker? - Android Enthusiasts Stack Exchange

On another note, I had a countdown app/widget that was eating my battery and I never saw any indication in GSam that it was causing the problem. I know that app was the issue, because it was the only app that I had downloaded when my battery started rapidly vanishing. I deleted the app, and my battery life returned. I'm just not convinced that GSam always catches problems. Of course, maybe I just don't understand GSam well enough to use it effectively.
