Based on other discussions I've seen, I believe that's the VoLTE icon.

Not sure. You don't want to make use of VoLTE? It's supposed to be a lot better than CDMA.

go to settings> mobile networks> access point names. you should only be seeing 1 data access point and 1 mms access point. if u are seeing more than 1, remove the other that is not in use.
