What is up with my battery?

The last couple of nights, my battery hasn't been charging much at all. Then this morning it spiked, jumping from 39% charged to 100% in just a few minutes.

Update: Now it's back down to 37%.

My guess is it needs to be replaced. Is it removable?

What phone do you have and how long have you had it? Those are signs of a bad battery and the phone is getting improper readings of the battery.

You can see if things get better in the coming days. If not, it's time to replace the battery if it's removable.
