Why am I getting an error message when trying to install Lollipop?

Nexus 4 - Trouble Installing Lollipop

I got a Nexus 4 second hand today and it's trying to download lollipop, but shortly after download on restarting, it shows an error message which says something like 'root access may be lost' with two answers 'yes' and 'no'

should I send the phone back?

It sounds like the phone you bought had been "rooted" by its previous user. If you are in contact with the seller, you might want to confirm that, as well as what I'm saying here: You are getting a warning that downloading & installing the new OS may undo that rooting - do you still want to proceed? If you don't care about root access (if you don't know what that is then you don't want or need it), you can say Yes and proceed, although I've never done this and I'd advise a second opinion.
Alternatively, you can ask the seller to restore the phone to unrooted factory condition, or return it and buy a different, unrooted phone from somebody else.
