Why can't my Nexus 7 (2013) recognise OTG USB mass storage?

I have a Nexus 7 and I have an OTG adapter (with MicroSD) connected to it but it doesn't even acknowledge that it's plugged in. I've been on this case for about 10 hours now. I've rooted the tablet and added the init.usb file to the system folder, I've used StickMount (active mounts none), Nexus Media Importer doesn't recognise it and my OTG Troubleshooter ticks the first two boxes to say the OTG was detected but No USB Device detected in Linux/Android (i.e. the Kernel does not see the device). A hardware issue, you say? Well, I thought the same until I plugged it into my Sony Xperia Z2 and it worked perfectly. I know it's going to be something silly or an option somewhere. Help would be really appreciated, thanks.
 
I see you haven't had any reply's and it so happens I have the same question. My NMI worked perfectly for six months and for no reason, at least from my view point, it won't detect my USB Device. I have been having this problem for two months and have tried more things then I care to remember with spotty results. It starts then shuts off, starts then off ETC>ETC. Like you said it's probably something I did like change a setting but I can't figure it out. Any advice? Did you get yours to work? Thanks
 
Mine is 100% stock, unrooted and mounts USB flash drives just fine:
ES File Explorer, Side Menu > Local > USBxxx
