Why do my contact widgets change appearance? [OnePlus 2]

[AC Question]

Hi all,

Having switched from IPhone to a OP2, I'm not that familiar with Android.
I have my most used contacts as a widget on the homescreen. I can see the contacts image, but after 3-5 days the image on the widget have been replaced with the green android robot. If opening the contact, the image hasn't changed though.

Anyone knows how to fix this?
