Why does my battery charge fully quickly then all of a sudden it has a low battery?

why my battery easily full charge then suddenly low battery status

this is what's happening in my tab 2:
1. i charge it, then suddenly it became full. afterwards its returns to low battery status (i observed this during charging, w/o using it)
2. i charged it for some time maybe 4 hours. it became full battery so i unplug it from charger. then after a few minutes it became low battery again w/o using it.
3. my battery is in low battery status. dont have my charger at the moment so i continue using it. then suddenly it become full battery status. but after a few minutes it will shut down.
my device is 2 years old now.

If you didn't make any changes on the rom then it is time to change new battery.

this happens to me exactly the same. my ROM is upgraded to 16gb.. i've tried using other original samsung charger from my sister but its still same problem.
