Why does my Galaxy S4 speaker default to on when making or receiving phone calls?

galaxy s4 speaker defaults to on when making or receiving phone calls

My phone is defaulting to speaker for incoming & outgoing calls and I don't know why. It just started doing this and I can't find the setting to stop it.

Welcome to Android Central! Look in the S Voice settings, and make sure Drive Mode is turned off.

How about Settings>Accessibility? Anything there turned on?
