why is my galaxy s5 active stuck in safe mode?

After I allowed an update my phone is stuck in safe mode I have tried following instructional videos from YouTube and nothing works, including rebooting and taking out the battery for 10 minutes. Also my app icons have changed to grey and won't open. Many of them won't even show up even though the app manager says they are installed.

Since the problem was evidently caused by the update, bring the phone to your carrier and ask them to please fix what they broke. (Make sure you back everything up first - see Backing up an Android Device.)
