Why is my Galaxy S5 Camera blurry past 6 away from target?

My Galaxy S5 Camera is blurry past 6" away from target

My galaxy s5 has a terrible rear camera. It constantly takes blurry photos and is more or less unusable. I had believed this was a result of scratches on the camera lens cover, but even after replacing this, it still takes the same blurry photos. Does anyone know how I could fix this or what I should do? Thank you for your responses.

If it has never focussed properly, it may be that the camera module is defective.
