why is my new samsung battery swollen?

I just got a new, Samsung approved, battery 4 months ago and it has already become swollen and loses its charge quickly. I dont over charge it and I use a Samsung snartphone charger on it, what is going on and are there cheap/good quality replacements?
 
The battery is defective -- don't use it any more, since there's a risk for fire or explosion. Contact the seller and see if they can exchange it for you.
 
should have worded that better... when I took it in last just to make sure it wouldnt do that again, before the swelling happened I was told, and I quote "the battery seems fine, its the right one (it was from samsung and everything), your cable is the correct brand and there shouldnt be any reason for worry and this one should last you for a long time" then not even a month later and it swells... and I have been checking. Asked several phone battery replacement stores. so frustrating. Could my phone itself be an issue? Its a 2013 samsung galaxy s4 and the battery it came with lasted me 3 years. The replacement I got is the same battery as the original.
 
Even OEM batteries can be defective, so this isn't all that unusual. I'm glad they'll replace it for free!
