Why is my Note 5 volume adjusting itself to minimum?

My Note 5 ringtone volume is adjusting itself. I constantly turn the volume up on my volume, I have updated it several times. I want to remove at least the last two updates.

Welcome to Android Central! Does this only happen when headphones are plugged in? If not, then it's possible the Vol Down button is faulty or stuck in a pressed state -- bring it to a repair shop to diagnose further.
