Samsung Galaxy Tab 2.7.0 P3100 Restart Automatically

I Got My tab Few year Ago Now For 5 Months My Tab Is Restart Automatically
I Factory Reset My Tab Many Time But Still it Is Not Working Then I Rooted My Device And Tried A App Call Phone Patcher (Phone Patcher Is Removed From Play Store I Got This From A Website) I Patched My Device But Few Months Later Now My Device Is Restarting Everytime I Plug My Charger
I am On The Stock And Rooted With CWM Recovery
