Every few days my phone stops connecting to my home wifi, my computers/ps3 still connect fine. When I restart my router/modem my phone will connect again no problem until a few days later.

This is frustating.

Same here. No idea what goes on. I've given up at this point as I get good 3G at my house but it would be nice to know if others are experiencing and if they found a solution.

Download WiFi analyzer from the app store for your phone, Open it up and see if there are any other wifi netoworks that are on your same channel. Your phone may see other networks on the same channel and be droping your router to try to pick up another network that it may precieve as stronger.

Or go into menu-->Settings-->Wireless and Networks-->WiFi Settings-->Menu Button-->Advanced and click on WiFi sleep policy. Select never. Sometime my phone will get confused and not reconnect when I get home so I have to turn WiFi off then back on, but it hasn't done it in a while.

Had this problem except my phone would screw up entire router. Bought a new router and now everything works perfectly

I have a bunch of junk SSID's in my WiFi list so occasionally I have to tell mine to connect to a specific one, until I delete out the junk connections. Try, Menu --> Settings -> Wireless & networks ->Wi-Fi settings -> Select a not in range "junk" network and it will ask if you want to Forget the network.

downloaded the wifi analyzer, non of the other networks are on my channel. So I reset modem/router again and it instantly connected. So now I have turned off the sleep mode and I guess well see if it messes up in a couple days or not.
