Wireless charging orb survey

I posted this on a Google+ Nexus 4 community page, but wanted to get more data on the wireless charging orb from current owners.

The survey below has four quick questions for those that have the orb. Whether or not your phone slips down while charging, please take this quick survey and perhaps we'll get a better idea of how prevalent slippage issues are and what conditions might contribute to it.
