On my iPhone 5, why can't I hear the caller and the call can't hear me unless I put on the speaker phone?

i phone 5 - I cant hear caller and caller cant hear me unless i put on speaker phone

I have just received new I phone 5
when I am speaker to someone after a few seconds they cant hear me - I have to put the phone on speaker then they can hear me - why?
