Why is media storage empty?

Galaxy S6, latest updates.

The other day I noticed no music files or image files in music player / image gallery. Don't know why it happened - the only thing I can think of is that the latest phone updates had recently been installed but don't know if this was the definite change point.

Files exist in Music & DCIM folders as expected, just not in the library. If I take a new camera photo, it shows up in Gallery, and in the DCIM/Camera folder (with the other pictures).

Have tried clearing data in media storage (stop service, clear cache, clear data, reboot) - doesn't help.
Have tried an app to rescan the library - doesn't help
Have moved all images out of DCIM/Camera folder and put them back - doesn't help

Am out of ideas now, can anyone offer any help?

Ok, this was me asking before I signed up!

It turned out to be the .nomedia file in the root of the storage folder (/storage/sdcard0) I thought I'd already looked for this, but obviously not hard enough. Deleting and rebooting fixed it. Still not sure which app created the file yet, but that's the next task if necessary!
