true 100% hands free make/receive calls for person with MS?

I have yet to find a smartphone or app to help my neighbor with receiving and making calls without some manual button pushing to get started or re-start every 5-30 minutes.
Even the always on smartphone/app seems to time out.
Found a $1200 app with a nexus phone online but he can't afford that.

Well always listening will only be continuously active if it's on charge, so you could do that.

What app or what phone?
Went to local T-mobile phone and tried every phone on display(each had charger hooked up) but "ok google" stopped working on each after 10-20 minutes. Even phones with travel mode.

Welcome to Android Central! In the Google app's Voice settings, under 'OK Google' detection, the Always On option is supposed to work "whether the screen is on or off" (which is the description in the menu itself). Those other phones you tried might have had some kind of power-saving mode turned on, which could interfere with this function.

But this will only be helpful for placing calls. I don't think there's an 'OK Google' command to answer an incoming call. You might have to try one of the auto-answer apps available on Google Play.
