?Copy both SD folders to New Card?

I am going from a 16GD SD card to a 32GB. The way I understand the procedure is that after removing the old card I have to mount and format the new one. My uncertainty is:

From the old card do I save and then copy to the new card both the SD and SD_EXT or only the SD_EXT folder? Thanks in advance for your assistance.

The SD_EXT is the micro SD card. SD is an internal storage on the phone. If you are swapping physical micro SD cards, you only need to copy the SD_EXT.
