I bought an Alcatel Onetouch Pop Icon here in Canada for about $120 new from the Telus store. It's a full-feature smart phone, you can use your stored stuff to restore your stuff on it (but memory is small, 32GB). It was good, I used it for about 6 months until the Note IV came out.

What have you tried? I'd try the last four digits of your phone number, or 0000 or 1234. It's also possible that it's the pass-sequence for your Samsung account. From the Note IV user manual: "If the unlock code is forgotten, take the device to a Samsung Service Centre to reset it."
