Low radiation app starts whle making phone calls

I started getting suspicious app "Low Radiation" while making phone calls. It comes up for 10-15 sec than disappears. It is annoying as while calling my voice mail I am getting message in Chinese (likely). Cannot find the app to uninstall. The phone is Samsung Note 2, rooted. Help is appreciated!

It comes from the DX Toolbox app. Latest update (I believe it's update 3.25) automatically enables the low radiation message on all incoming and outgoing calls. Simply open up the DX Toolbox app and go to the options tab (three vertical dots) in the upper right hand corner above "App Manager", then select "System Settings", scroll all the way down and you'll see an option called "Call radiation notice". Tick that and turn off the "all radiation notice" option.
