Restarting automatically when battery is less than 15%

My S4 is acting really weird. I did a factory reset a few days ago, and now the phone will automatically restart itself when the battery is 15% or below. Funny thing is that the restart never completes. When I plug the phone in to charge the battery indicator shows zero battery. A related issue is that the battery drains way faster than normal even though I've done the OTA update to increase battery life and turned off/disable background apps. \

Does anyone have a fix for this issue?

P.S. It just died on me right now while watch an episode I downloaded and saved on my SD card. The battery was at around 45% when I started watching it and was flat within 20 minutes
