DO you have to? No. there are other recovery options.
Will you lose your data if you don't? yes the other options usually mean a fresh install

Now should you? YES YES YES. It will make life so much more easy if something goes wrong.

It really depends on the circumstances.

Some ROMS allow you to just flash over the pre-existing one.

I recommend doing nandroid backup of your ROM before flashing a new one. This way you can always revert back if you don't like the new one or if there is a problem with theme one.

Back up apps with Titanium...it's worth the money.
