Lost contacts icon on Samsung Galaxy s5

[AC Question]

I installed a screen rotation app and lost icons, but when I uninstalled it, I managed to get the icons all back except the Contacts. It appears in my App Manager list and appears in Settings - Applications. It is not disabled and it was not moved to the SD card. When I long press on home screen to add the contacts widget, it wants me to select only one contact to call. If I don't select one contact, it doesn't go on the home screen. I've seriously tried so many things and now I need help!

did you do a software update recently? If so, what version of Android do you have?
