Why am I getting phantom notifications on my Note 4?

I have disabled notifications from all the apps that I can think of yet I still receive a notification sound or vibration (when on silent) about 4 or 5 times a day. Verizon couldn't figure it out and gave me a new phone. Still the problem persists. There is no visible notification indicator when the phone notifies me. Pretty annoying especially when my phone vibrates for no reason at 1am.
