why is my phone freaking out?

[AC Question]

I ATTEMPT to turn on my phone and it doesn't turn on then when it does it must be connected to a charger and then when its on only lasts for about 5 minutes MAX. and when it shuts off by itself a picture appears saying 'Power Reset or Unknown Upload Mode' with the android icon/mascot standin next to it and then I have to take out the battery.

Welcome to Android Central! Which phone? I suspect a failing battery--try a new one.
