Do you want to remove just the app? Or all of the Google Play services? To completely remove everything would require root access and a force removal of several components above and beyond the Google Play Store app. This can have some unintended consequences as once you go in and remove these components, you also run the risk of modifying the system installation enough to not be able to take another OTA update.
The only effective way, in my opinion, is to remove Google Services is to install a custom AOSP or CM ROM that does not include any Google Play Services in the default image. Doing so app by app and service by service, there's a very good chance you'll corrupt your phone's software, requiring you to reload the default factory image to fix.
