"Unfortunately phone has stopped"

[AC Question]

Galaxy s5. Whenever I have ANY application open and I receive a call, I get the message: "Unfortunately phone has stopped"! And the ONLY way to get out of it is to restart my phone in between pressing "ok" on the message and pressing "restart phone" before the message reappears. It just started happening. Nothing was added or removed from phone! It does NOT happen when I'm am not in any application. (I've emptied all caches, and it still happens)

I have the exact same issue on my Note 4 for the past couple of months. I was hoping the latest Android update would address the issue but it did not.

Does anyone have to solution to this problem?
