Saved/Bookmarked page with no or invisible favicon

I saw a page I liked, so I bookmarked it and had it saved to "Add To Home Screen". I have done this many times. I have an LG smart phone.

When I booknarked it, I changed the name to something shorter that will fit on one line on the folder. Did this many times as well.

The problem is I only see the folder name and not the favicon. Viewing source there is no code to have a favicon. Usually if this happens, no favicon, it defaults a folder name starting with just one letter.

How can I redo or add a visible folder icon?
