I need to hard reset my galaxy note 4 every 3-4 weeks

After the hard reset the phone works fine for 3-4 weeks, then startint to freezes and lagging so bad that i need to reset it again in order to work, and this is the second phone, i replaced 2 months ago and still is doing the same thing, i can't even pick up a call or make a call, screen goes black, like a memory issues, hopefully 5.1.1 will resolved the issues, i already did a back up using kies to reset it again tonight and then restored the data

Most likely it's software related. Are you using automatic backup and restore?
