How does this app work or Amy kind of notifier work? Like for gmail it knows how many unread messages I have does that mean the developer if he wanted can open up those emails and read them?

The app could. In some versions of Android. It doesn't. It's looking at a count of unread messages, it's not looking at the messages. (You can ask a database for a count without being told anything else.)
