App Icon Switched to Android Robot

Hi, I have a Samsung E tablet. Since last week one of my app icons switched to the little green android robot. I still cannot figure out why this has happend. I have tried uninstalling the app and reinstalling it and the app comes back as a green robot. This app is not on my home screen, just in my app screen. Can someone please help me solve this issue? Thanks!

Welcome to Android Central! Do you know if the app had been Moved to SD at some point (in the Settings>Apps​ menu)? If so, that can happen if the card is corrupt.

I do not have an SD card in the tablet.
