Be glad you can't get the update. I have a note 4 running on AT&T and this has been the worst Android update I've ever experienced. Phone ran great on 4.4.4. After the Lollipop update:
Phone randomly calls people just for the hell of it....as in I can be playing a game and the game minimizes and calls a random contact
Phone is sluggish. At times it can take over 3-4 seconds for the home button to respond and close the active app
Icons keeps disappearing. The text will be there (i.e. outlook, gmail, phone) but the icon is completely missing.
Does anyone know of any fixes for any of the above issues?
There are a lot of other threads with answers to your questions. Most people have had no problem with the update (including me), but a bad app (not optimized for Lollipop) could be the culprit.
And you could also try this (it does no harm; you do not lose any info; and it doesn't matter what carrier you are on):

And (as a last resort) you could also do a factory reset, but you will have to set up your phone from scratch again.
EDIT: For the missing icons you can try this:
Go to Settings>Application Manager>All>Scroll down to the Keeper App>Disable it and then restart phone.
